Clinical Observation of Civilian Colorectal Injuries



Abstract
The records of 87 traumatized patients with either blunt and/or penetrating colorectal wounds, treated between January, 1982 and December, 1990, were retrospectively reviewed.
A majority of the injuries were about to blunt injuries(52 cases, 59.8%) from traffic accident (38%) and violence(12%).
Injuries occured most commonly in the transverse colon(41.2%), followed by the signoid colon(15.9%), and rectum(13%).
The patients, predominently young and male, were cften victims of trauma or stabbings. Associated intraadbominal injuries were frequent(55cases, 63.2%).
By operative procedure, primary closure or resection anastomosis of colon(Group A) were 56 cases(65.5%), primary closure with exteriorzation(Group B) were 6(6.9%) and resction of injured colon with end colostomy or mucous fistula or primary
closure
with
proximal diversion (Group C) were 25(27.6%) cases.
The complication rate was 41.4%(36 of 87 cases) and the major complications were wound infection921.8%), leak of primary suture(11.5%), wound dehiscence(8.1%), intestinal obstruction(6.9%) and intraabdominal abscess(5.8%).
According to operative procedure. Group a complications were 19 cases, group B were 3 cases, Group C vere 14 cases.
Overall mortality was 6.8% and included 6 patients who died form sepsis(3cases) and hypovolemic shock(3cases).
